BASIC EDUCATION ADMISSION REQUIREMENTS

3.   REQUIREMENTS:

1.1.  Pre-Kindergarten

                a. Photocopy of PSA Birth and Baptismal Certificate

                b. At least 4 years old by October

                c. Two (2) pieces 1×1 recent colored ID pictures

                d. One (1) piece White Folder with fastener

d. Interview with K to 10 Coordinator, Guidance Counselor, and School Principal          

1.2.  Kindergarten

Must be 5 years old by October 1 (Deped ORDER No. 16 s. 2015)

a. Photocopy of PSA Birth and Baptismal Certificate

b. Two (2) pieces 1×1 recent colored ID pictures

c. One (1) piece White Folder with a fastener

d. Interview with K to 10 Coordinator, Guidance Counselor, and School Principal

1.3      Grades 1, 7 and 11

a. Form 138 (Report Card) with no failing grade within the curriculum year

b. Form 137 (from the previous school attended)

c. Accomplish ESC Form & submit the required documents

d. ESC Certificate (for Incoming Grade 11 from ESC Participating Schools)

e. Photocopy of PSA Birth Certificate  and Baptismal Certificate

f. Certificate of Good Moral Character from the School Principal and/or Guidance    Counselor of the  school previously  attended

g. A satisfactory rating in the Entrance Examination administered by the school.

h. Interview with K to 10/SHS Coordinator, Guidance Counselor, and School Principal

I. NCAE result for incoming Grade 11 students (if applicable)

j. Two (2) pieces of 1×1 recent colored ID pictures

k. One (1) piece White Folder with a fastener

1.4      Old Students

a. Form 138 (Report Card)

b. Accomplished Enrollment Form

1.5.  Transferees / Returnees

a.         Satisfactory results in the Entrance examination and interview

b.         A passing grade in the Final Rating in all subjects

c.         Certificate of Good Moral Character from the Principal and/ or Guidance Counselor of the school previously attended

d.         Photocopy of PSA Birth Certificate and Baptismal Certificate

e.         Certificate of Transfer  from the school previously attended

h.     ESC Certificate from the previous school attended (ESC participating School) for transferees in Grades 8-10 and 12 respectively.
