ENROLLMENT PROCEDURES

BASIC EDUCATION DEPARTMENT
For New and Transferee Pupils/Students
FOR WALK-IN:
Step 1 – K-10 Academic Coordinator / School Principal & SHS Academic Coordinator
Interview first with the School Principals with the required initial documents and an Enrollment Form will be given.
Step 2 – Guidance Office
Take the Entrance Examination and accomplish the Guidance Forms.
Step 3 – Prefect of Discipline
Present the filled-out enrollment form and have it singned.
Step 4 – Student Activity Coordinator
Present the filled-out enrollement form and have it signed.
Step 5 – Registrar’s Office
Present the filled-out enrollment form for processing and submit the required documents.
Step 6 – Cashier’s Office
Pay the required payment upon enrollment.
Step 7 – Registrar’s Office
Submit the completed enrollment form.
Step 8 – ID Center
Proceed for ID picture taking.
FOR ONLINE:
Step 1 – Open the enrollment link – https://sprl.in/ckc-new-student , click submit, and print the payment slip using the link provided below.
Step 2 – Secure signatures indicated in the slip and submit the enrollment documents required in the offices.
Step 3 – Pay the required payment upon enrollment in the cashier’s office.
Step 4 – Submit the accomplished payment slip in the registrar’s office.
Step 5 – Proceed for ID picture taking in the ID Center.
FOR OLD PUPILS/STUDENTS
FOR WALK-IN:
Step 1 – Registrar’s Office
Get the Report Card and secure the Enrollment Form
Step 2 – Guidance Office
Present the filled-out enrollment form and secure the signature.
Step 3 – Prefect of Discipline
Present the filled-out enrollment form and secure the signature.
Step 4 – Student Activity Coordinator
Present the filled-out enrollment form and secure the signature.
Step 5 – K-10 Academic Coordinator / School Principal & SHS Academic Coordinator
Present the filled-out enrollment form with your report card (Form 138) for academic Consultation.
Step 6 – Registrar’s Office
Present the filled-out enrollment form for processing.
Step 7 – Cashier’s Office
Pay the required payment upon enrollment.
Step 8 – Registrar’s Office
Submit the completed enrollment form.
Step 9 – ID Center
Proceed for ID picture taking.
FOR ONLINE:
Step 1 – Open the enrollment link – https://forms.gle/18szkXJati1rk8nF6, click submit, and print the payment slip of the link provided below.

COLLEGE DEPARTMENT
For New, Transferee and Returnee Students
Step 1 – Dean of Student Affairs
Interview with the Dean of Student Affairs with the required initial documents and an Enrollment Form will be given.
Step 2 – Guidance Office
Take the Entrance Examination and accomplish the Guidance Forms.
Step 3 – Program Heads’ Office
Interview and Present the Test Result, Grade Evaluation/Transcript of Record (TOR)/ Honorable Dismissal. Plot the subjects in the Registration Form.
Step 4 – Dean of College
Secure signature.
Step 5 – Registrar’s Office
Present the Filled-up Registration Form for processing and encoding of subjects
Submit all the Admission Requirements.
Step 6 – Cashier’s Office
Pay the required payment upon enrollment.
Step 7 – Registrar’s Office
Submit the accomplished enrollment form for printing of assessment.
Step 8 – Dean of Student Affairs
Secure Student Handbook.
Step 9 – ID Center
Proceed for ID picture taking.
For Old Students
Step 1 – Registrar’s Office
Secure the Enrollment Form
Step 2 – Accomplish the Enrollment Form
Step 3 – Cashier’s Office
Pay the required payment upon enrollment
Step 4 – Registrar’s Office
Submit the accomplished enrollment
Step 5 – ID Center
Proceed for ID picture taking
